The Short Answer
Gluten is a protein found in three grains: wheat, barley, and rye. It gives dough its stretch and bread its chew. For most people it is harmless. For people with celiac disease, eating it triggers an immune reaction that damages the small intestine — which is why avoiding it completely is the only treatment.
If you need to avoid gluten, the three grains above are what you are avoiding, along with anything made from them and anything they have contaminated.
What Gluten Actually Is
"Gluten" is not a single substance. It is a family of storage proteins that grains use to feed the growing seed. In wheat, the two main ones are gliadin and glutenin. When flour is mixed with water and worked, these proteins link together into a stretchy, elastic network — that network is what we call gluten.
That elasticity is why gluten matters in the kitchen. It traps the gas produced by yeast, letting bread rise and hold its shape. It is what makes pizza dough stretch and pasta hold together. Without water, gluten does not form at all; without kneading, it stays weak.
Barley and rye contain closely related storage proteins — hordein in barley, secalin in rye. They are different molecules from wheat gliadin, but similar enough that the immune system of someone with celiac disease reacts to all of them. For that reason, all three grains are grouped together under the single word "gluten."
Which Grains Contain Gluten
The three primary gluten grains are wheat, barley, and rye. Several other grains belong to the wheat family and contain gluten too:
- Spelt — an older type of wheat, sometimes sold as a "healthier" alternative. It is still wheat.
- Kamut (khorasan wheat) — another ancient wheat. Also contains gluten.
- Triticale — a hybrid of wheat and rye, so it has both.
- Farina and semolina — these are wheat, milled differently.
- Malt — almost always made from barley. Malt, malt extract, malt syrup, malt vinegar, and malt flavoring all carry gluten.
A grain being ancient, organic, or "wheat-free" does not make it gluten-free. Spelt and kamut are wheat. And "wheat-free" is not the same claim as "gluten-free" — a wheat-free product can still contain barley or rye.
The Oats Question
Oats are the exception that causes the most confusion. The oat plant does not contain wheat, barley, or rye gluten. In its pure form, oat protein is safe for most people with celiac disease.
The problem is not the oat — it is the supply chain. Oats are routinely grown in rotation with wheat and barley, harvested with the same equipment, and milled in the same facilities. By the time ordinary oats reach the shelf, most are contaminated with gluten grains. That is why regular oatmeal is not safe for celiac disease, even though the oat itself is naturally gluten-free.
The fix is to buy oats specifically labeled gluten-free. These are either grown under a "purity protocol" — kept separate from gluten grains at every stage — or mechanically sorted and tested to stay under the legal gluten limit. Look for that label, not just the word "oats."
One more wrinkle: oats contain a protein of their own called avenin, structurally similar to gluten. For most people with celiac disease, avenin causes no reaction. For a small minority, it does. Estimates of how many vary widely across studies. If you react to certified gluten-free oats, avenin is the likely reason — and worth raising with your doctor rather than assuming the oats were contaminated.
Where Gluten Hides
The obvious sources are bread, pasta, cereal, cake, beer, and anything breaded or battered. The harder part is the gluten you do not expect, because wheat and barley turn up as ingredients in foods that are not "bread."
Common hiding places:
- Soy sauce — traditionally brewed with wheat. Many soy sauces are not gluten-free; tamari is often the safer alternative, but check the label.
- Sauces, gravies, and soups — wheat flour is a standard thickener.
- Salad dressings, marinades, and some mustards — may use malt vinegar or wheat-based ingredients.
- Processed meats, veggie burgers, and imitation seafood — often use wheat as a binder or filler.
- Malt in any form — barley malt, malt extract, malt flavoring, malt vinegar. Shows up in cereals, candy, and drinks.
- Modified food starch and dextrin — usually corn-based in the US, but can be wheat-derived. Check.
Reading the ingredient list is the reliable defense. In the US, if a product contains wheat, the label must say so — but barley and rye are not required allergen declarations, so they can appear only under names like "malt." When in doubt, look for a gluten-free label rather than trying to decode every ingredient.
What "Gluten-Free" on a Label Means
In the United States, "gluten-free" is a regulated claim. Under the FDA rule, a food carrying it must contain less than 20 parts per million (ppm) of gluten — the threshold research and regulators settled on as safe for most people with celiac disease.
A few things that follow from this:
- A product can contain a specially processed ingredient like gluten-free wheat starch and still be labeled gluten-free, as long as the finished food tests under 20 ppm.
- Third-party certification (such as GFCO) applies an independent test on top of the manufacturer's own claim. It is an extra layer of assurance, not a legal requirement.
- "Gluten-free" is not the same as "zero gluten." It means below a threshold, not absent. For most people with celiac disease that threshold is safe, but a very sensitive minority react below it.
Why Gluten Causes Harm — Briefly
For most people, gluten is just food. The reason it harms some people comes down to how the body responds to it, not to the protein being toxic in general.
- In celiac disease, gluten triggers an autoimmune reaction that damages the lining of the small intestine over time. It is a lifelong condition, and the only treatment is a strict gluten-free diet.
- In non-celiac gluten sensitivity, gluten causes symptoms without the autoimmune damage seen in celiac disease. It is less understood and diagnosed by exclusion.
- In wheat allergy, the immune system reacts to wheat proteins the way it would to any other allergen. This is a true allergy, separate from the other two.
These are three different conditions with three different mechanisms. If you think gluten is affecting you, get tested before cutting it out — going gluten-free first can hide the very thing the tests are looking for.
